Job summary

University Hospitals in Mentor/Concord area seeks a Social Worker for their Seidman Cancer Center. This outpatient, full-time dayshift role supports oncology patients and families, coordinating psychosocial assessments, discharge planning, and community resource linkage with a multidisciplinary team.

Requires a Master’s Degree in Social Work and Licensed Social Worker (LSW). LISW is preferred within one year.

Qualifications

• Master's Degree from an accredited school of Social Work (Required).
• Licensed Social Worker (LSW) (Required).
• Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility).
• Hospital/Health Agency experience (Preferred).
• Theoretical and clinical knowledge regarding health care delivery (Required).
• Multi-tasks and prioritizes work (Required).
• Works autonomously (Required).
• Communicates effectively with persons of various backgrounds (Required).

Responsibilities

• Collaborates with health care team, patient and family to assess psychosocial and discharge planning needs.
• Provides psychosocial assessment/intervention for clinically complex patients and families.
• Develops and documents comprehensive discharge plans.
• Links patients to community resources and UH network services.
• Acts as consultant on psychosocial and discharge planning issues/barriers.
• Assists with recruitment, orientation, and mentoring of staff and interns.
