Qualifications

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education.
• Dosing Recommendations: Determine optimal doses based on age, weight and organ function.
• Collaborate with hematology/oncology teams for inpatient and outpatient care.
• Oversee preparation of chemotherapy and investigational medications.
• Attend rounds and provide pharmacy services in interdisciplinary care.
• Develop policies and safety initiatives for oncology patients.
