Job summary

Carle Health seeks a Pharmacist to deliver comprehensive pharmaceutical care for oncology patients within the Carle Cancer Center. You will support specialty care across multiple departments and participate in clinical trials, MTM, and policy development.

The role emphasizes direct patient care, dosing optimization, and collaboration with hematology/oncology teams, with inpatient and outpatient responsibilities and a focus on safety and quality.

Qualifications

• Licensed Pharmacist per IDFPR (Illinois)
• Bachelor's Degree: Pharmacy is required
• Clinical work experience in a hospital/clinical setting

Responsibilities

• Direct Patient Care: intervene to optimize prescribing, administration, and monitoring.
• Provide MTM services and patient education as defined by pharmacy practice acts.
• Determine dosing based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams (inpatient and outpatient).
• Prepare investigational and standard medications for clinical trials.
• Attend rounds and provide pharmacy services in interdisciplinary care.
• Collaborate with providers to improve patient-centered care.
• Oversee preparation of chemotherapy and monitor therapy-related adverse effects.
• Assist with transitions of care and staff development.
• Develop and promote oncology-related policies and safety initiatives.
