Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring within the facility.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in applicable regulations.
• Dosing Recommendations: Determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for inpatient and outpatient care.
• Prepare chemotherapy safely for clinical trials and standard care.
• Attend rounds and provide pharmacy services in interdisciplinary teams.
• Develop medication policies and participate in safety initiatives.
