Job summary

Qualifications

• Licensed Pharmacist (IDFPR) is required.
• Bachelor’s Degree in Pharmacy is required.
• Clinical work experience in a pharmacy setting is required or preferred.

Responsibilities

• Direct Patient Care: intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): provide MTM services and patient education as defined by policy.
• Dosing Recommendations: determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams, for inpatient and outpatient care.
• Prepare investigational and standard chemotherapy for clinical trials and ensure safety.
