Job summary

Carle Health seeks an experienced Pharmacist to deliver comprehensive oncology pharmaceutical care within the Carle Cancer Center. The role supports specialty care across multiple disciplines and serves as a clinical expert for inpatient and outpatient oncology services.

Key duties include direct patient care, MTM, dose optimization, chemotherapy preparation, and collaboration with hematology/oncology teams.

Qualifications

• Licensure as pharmacist (IDFPR)
• Pharmacy degree (Bachelor’s) required
• Clinical work experience in pharmacy settings

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined.
• Dosing recommendations based on age/weight/renal/hepatic function.
• Collaborate with hematology/oncology & inpatient pharmacy teams (inpatient & outpatient).
• Prepare investigational and standard of care medications for clinical trials.
• Attend rounds and provide pharmacy services in hematology/oncology.
• Collaborate with providers to enhance patient-centered care.
• Monitor and manage adverse drug effects and adherence.
• Oversee chemotherapy preparation; support transitions of care.
• Contribute to policy/procedure development and safety initiatives.
