• Provide MTM services and patient education as defined in relevant pharmacy practice guidelines.
• Determine optimal dosing based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for both inpatient and outpatient care.
• Oversee preparation of chemotherapy and investigational medications.
• Attend rounds and provide pharmacy services in oncology settings.
• Develop and promote medication policies, procedures, and safety initiatives.
• Supervise pharmacy technicians and contribute to staff development.
