Job summary

Bachelor’s Degree: Pharmacy (Required).

Responsibilities

Provide direct patient care to improve prescribing, administration, and monitoring.
Deliver MTM services and patient education per Pharmacy Act.
Determine dosing based on age, weight, and organ function.
Collaborate with hematology/oncology and inpatient pharmacy teams.
Prepare investigational and standard chemo for trials.
Attend rounds and provide services in interdisciplinary care.
Support policy development and safety initiatives for oncology patients.
Oversee dispensing of medications and chemotherapy preparation.
Supervise technicians and assist in staff orientation.
Participate in committees, audits, and drug usage evaluations.
