Job summary

Carle Health seeks a licensed pharmacist to provide comprehensive pharmaceutical care to oncology patients and support specialty care across multiple departments. You will participate in MTM, dosing optimization, and chemotherapy preparation while collaborating with hematology/oncology teams and supporting clinical trials.

Responsibilities include direct patient care, medication safety, and contributing to policy development.

Qualifications

• Licensed Pharmacist with IDFPR certification
• Bachelor’s degree in Pharmacy (Required)
• Clinical work experience

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices within the facility.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
• Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for both inpatient and outpatient responsibilities.
• Prepare investigational and standard-of-care medications for clinical trials.
• Attend hematology/oncology rounds and provide pharmacy services in the interdisciplinary practice at Carle Foundation Hospital.
• Collaborate with hematology/oncology providers to enhance patient-centered care.
• Oversee and direct the preparation of chemotherapy.
• Assist with transitions of care and staff development.
• Develop medication-related policies and promote safety initiatives for oncology patients.
