Job summary

Carle Health is seeking a licensed pharmacist to join our Oncology Pharmacy team. You will deliver comprehensive pharmaceutical care to oncology patients and support specialty care across multiple disciplines within Carle Cancer Center.

The role emphasizes MTM, dosing optimization, and collaboration with hematology/oncology teams, with responsibilities in inpatient and outpatient settings and participation in trials and safety initiatives.

Qualifications

• Licensed Pharmacist with IDFPR.
• Bachelor’s Degree in Pharmacy required.
• Clinical work experience required.

Responsibilities

• Direct patient care: Intervene to improve prescribing, administration and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined by regulations.
• Dosing recommendations based on patient age, weight and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for both inpatient and outpatient care.
• Oversee safe preparation of chemotherapy and investigational meds for trials.
• Attend rounds and provide pharmacy services in interdisciplinary care.
• Develop medication policies and promote safety initiatives for oncology patients.
• Participate in pharmacokinetics dosing service activities and hospital committees.
