Cooper Norcross Health is seeking a Patient Navigator to support clinicians and patients by coordinating scheduling, pre-certification, and follow-up tasks. The role focuses on ensuring continuity of care and timely appointments within the Camden, NJ health system.

Ideal candidates have healthcare office experience, knowledge of inpatient/outpatient settings, and strong interpersonal skills. High school diploma or equivalent with clinical training is preferred; flexible, team-oriented

• Experience in a healthcare office setting.
• Knowledge of inpatient/outpatient settings and oncology population.
• Strong interpersonal skills with diverse backgrounds.
• Ability to take direction and follow through.

Responsabilités

• Bridge the gap between clinician and patient by providing non-clinical support in navigating the health care system for continuity of care.
• Coordinate scheduling, pre-cert processes, follow-up calls and record retrieval for timely appointments.
• Call new patients within 24 hours of visit to check in regarding follow-up appointments and needs.
