Job summary

New England Cancer Specialists is seeking a part-time 24-hour Palliative Care Nurse Navigator in Lewiston, ME. This role focuses on direct patient care for Hematology/Oncology patients in an outpatient setting, collaborating with an interdisciplinary team to improve symptom and pain management.

The ideal candidate has a BA/BS, Maine RN license, and 2+ years in palliative/hospice nursing. Oncology experience and hospice certifications are beneficial.

Qualifications

  • Bachelor's degree in nursing required.
  • Licensed Nurse within the State of Maine is required or ability to obtain.
  • 2+ years of palliative/hospice nursing experience required.
  • Hematology/Oncology experience preferred.
  • Palliative, Hospice, Supportive Nursing Experience preferred.
  • Hospice Palliative Nursing Care Certification preferred.
  • Ability to communicate effectively with patients, physicians, and staff.
  • Must be able to manage multiple priorities.

Responsibilities

  • Coordinate care with interdisciplinary teams to manage symptoms and pain in oncology patients.
  • Draw blood from ports, central lines, and peripheral IV access as needed.
  • Follow up with patients after office visits on symptom management results.
  • Reach out to patients and families involved in palliative/hospice care.
  • Screen patients using dashboards and review hospitalizations for follow-up.
  • Handle script refills, prior authorizations, referrals to Hospice and Home Care.
  • Assist physicians and conduct patient assessments.
  • Document interventions and track patient/family responses per protocol.
  • Participate in team conferences, education programs, and relevant organizations.
