Qualifications

• Registered nurse in Maine with current licensure.

• Must have strong IV skills and patient-focused care.

Responsibilities

• Serve as point of contact for referrals, schedule and coordinate care.
• Perform patient assessments and triage needs by phone and in clinic.
• Deliver patient education and counsel; communicate patient needs to physicians.
• Monitor treatment adherence and follow-up appointments by phone or in person.
• Collaborate with physicians, APPs, RNs, and care coordinators to optimize care.
