Job summary

New England Cancer Specialists in Topsham, ME seeks a part-time Clinic Oncology Nurse Navigator to help patients and families navigate the health care continuum. The nurse will assess patients, provide advice and coordinate with physicians on patient needs and interventions.

The role involves nursing assessments, triage by phone and in-office, and collaboration with physicians, APPs, RN peers and care coordinators. Very minimal travel and occasional weekend coverage may be required.

Qualifications

• Bachelor degree in Nursing preferred; BSN enrolled within 6 months and complete within 4 years.
• Licensed Nurse within State of Maine required.
• OCN Certification preferred.
• Two years of Hematology/Chemotherapy/Oncology nursing experience preferred.
• ONS Chemotherapy & Bio-therapy Certification required or obtainable within one year.
• Two to five years plus nursing experience expected.
• Must have strong IV skills.
• Ability to communicate effectively with patients, physicians and staff.
• Must be detail oriented and able to manage multiple priorities.

Responsibilities

• Point of contact for referring physicians, patients, and caregivers to assist with patient scheduling and coordination of care.
• Assist physicians; conduct patient assessments.
• Receive patient telephone calls and intervene appropriately for that patient’s specific needs in a timely manner; alert physician as needed.
• Follow up with patients by telephone to ensure adherence to treatment guidelines and evaluate changes.
• Draw blood from ports, central lines, and peripheral IV access; perform other procedures as needed.
• Ensure follow-up appointments and tests/procedures are scheduled.
• Maintain patient care standards and ensure documentation is completed safely and effectively.
• Maintain open communication with physicians, employees and vendors.
• Participate in interdisciplinary conferences, in-services and continuing education in Oncology/Hematology.
