Job summary

Daymark Health is seeking a licensed Nurse Practitioner or Physician Assistant to join our care team delivering in-home and virtual oncology care in the Inland Empire. You will provide hands-on clinical care, coordinate triage for high-acuity patients, and support the full cancer care continuum with empathy and collaboration.

Qualifications

• Active NP or PA with at least 4 years of direct clinical care
• Oncology, home care, palliative care, hospice, or primary care experience required
• California BRN certification as 103/104 NP is required

Responsibilities

• Provide direct patient care in home and via virtual platforms; perform assessments, place orders, prescribe medications, document, and coordinate tasks for the care team
• Triage for high-acuity patients and collaborate with oncology practices via daily standups
• Support patients across active treatment, survivorship, and end-of-life care with a patient-centered approach
• Build trusting relationships with patients and care networks through empathy and active listening
• Collaborate with operations to refine and implement Daymark's care model and ensure timely, appropriate care for cancer patients
