Job summary

Daymark Health is redefining cancer care by delivering value-based, home-based oncology services in Orange County. This full-time NP/PA role focuses on hands-on clinical care in patients' homes and via virtual visits, building a high-performing, patient-centered care team.

You will triage high-acuity cases, advance the Daymark care model, and collaborate with operations to improve access, outcomes, and patient experience.

Qualifications

• Active NP or PA with at least 4 years of direct clinical care.
• Oncology, home care, palliative care, hospice, or primary care experience required.
• Oncology Nursing Society certification, value-based care and/or home care experience a plus.

Responsibilities

• Provide direct patient care in home and via virtual platforms.
• Triage high-acuity patients and collaborate with oncology practices.
• Support patients across the full care continuum.
• Build trusting relationships with patients and care networks.
• Refine and implement Daymark's care model with clinical operations.
