Job summary

Care New England is seeking a Nurse Navigator to work within the oncology multidisciplinary team in Rhode Island. The role focuses on advocacy, education, and coordination of comprehensive cancer care from screening through survivorship.

Responsibilities include guiding patients and families, delivering education, and coordinating with clinicians and community resources to ensure seamless care. RN licensure in Rhode Island and CPR are required; ONS certification is required; navigation

Qualifications

• Bachelor of Science in Nursing required with at least 5 years of nursing experience in Oncology
• Nurse Navigator experience required
• Current RN licensure in Rhode Island and CPR certification required
• Oncology Nursing Society certification required
• Nurse Navigation Certification preferred and/or obtained within 6 months of hire

Responsibilities

• Provide nursing care and guidance to cancer patients from screening to survivorship following the nursing process and standards of practice
• Educate patients and families to ensure seamless, comprehensive care and document teaching
• Respond to patient information requests about disease, treatment side effects, and resources
• Support patients during difficult decisions and coordinate end-of-life care as needed
• Coordinate with multidisciplinary team and community resources for timely, compassionate care and update policies as needed
