Job summary

Mercy South Sindelar Cancer Center is seeking a Registered Nurse Navigator to guide oncology patients and families through the cancer journey, coordinating care from diagnosis through survivorship. You will educate patients, address barriers to treatment, and collaborate with a multidisciplinary team to ensure timely, high-quality care with an exceptional patient experience.

Ideal candidates have 2–3 years of nursing experience, an RN license, BLS, and a BSN preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse license in the state of practice.
• 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Responsibilities

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effect management, and available support resources.
• Facilitate communication between patients, providers, infusion services, imaging departments, specialty providers, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and proactively identify and address barriers to care.
• Provide emotional support and connect patients and families with appropriate community resources, financial assistance programs, and support services.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
