Mercy South Sindelar Cancer Center is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide oncology patients and families through their cancer journey. You will coordinate care, educate on treatment plans, and support transitions from diagnosis to survivorship, closely collaborating with oncologists, infusion services, and social work.

The role emphasizes individualized care plans, timely communication, and reliable documentation in EPIC, while helping patients access resources

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years nursing experience.
• BLS certification.

Responsibilities

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families on diagnoses, treatment plans, medications, procedures, side effects, and support resources.
• Facilitate communication among patients, providers, infusion services, imaging, and care teams.
• Collaborate with physicians to develop and execute individualized care plans.
• Assist with referrals, appointment scheduling, and transitions of care.
• Monitor patient progress and address barriers to care.
• Provide emotional support and connect patients with resources and financial assistance.
• Maintain accurate documentation in EPIC and clinical systems.
• Track patient outcomes and quality metrics to support program development.
