Job summary

Mercy South Sindelar Cancer Center is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide oncology patients through diagnosis, treatment, survivorship, and follow-up care. You will coordinate care with oncologists, infusion services, social work, and other teams to deliver timely, high-quality care and an exceptional patient experience.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• 2-3 years of nursing experience.
• BLS certification required.

Responsibilities

• Coordinate and navigate oncology care across diagnosis, treatment, survivorship and follow-up.
• Educate patients and families on diagnoses, treatment plans, medications, procedures and support resources.
• Facilitate communication among patients, providers, infusion services and care teams.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Assist with referrals, appointment coordination and transitions of care.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track patient outcomes and support program development.
