Job summary

Mercy South Sindelar Cancer Center is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide oncology patients and families through the cancer journey. You will coordinate care, educate patients, remove barriers to treatment, and work with oncologists, infusion, imaging, social work, and other teams to ensure timely, high-quality care and exceptional patient experience.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• 2–3 years nursing experience.
• BLS certification.

Responsibilities

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effects, and resources.
• Facilitate communication between patients, providers, infusion services, imaging, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Assist with referrals, appointment scheduling, and transitions of care.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track patient outcomes and quality metrics to support evidence-based practice.
