Mercy South Sindelar Cancer Center is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide oncology patients and families through the cancer journey. You will coordinate care, provide education, and collaborate with a multidisciplinary team to ensure timely, high-quality care and an exceptional patient experience.

Competenze

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification.
• BSN preferred; OCN certification preferred; ACLS certification preferred.

Mansioni

• Coordinate and navigate care for oncology patients from diagnosis through survivorship.
• Educate patients and families on diagnoses, treatments, meds, and side effects.
• Facilitate communication among patients, providers, and support teams.
• Develop and execute individualized care plans with physicians.
• Assist with referrals, scheduling, and transitions of care.
• Maintain accurate documentation in EPIC and other systems.
• Monitor outcomes and identify barriers to care.
• Provide emotional support and connect to community resources.
• Support quality metrics and program development.
• Serve as clinical resource for patients, families, and staff.
