OneOncology is seeking a Nurse Navigator to coordinate cancer care and provide patient-centered education across the care continuum. This is a full-time role based at our Muskegon clinic.

The nurse will guide patients through diagnosis, treatment options, appointments, and support services while ensuring timely access to care and adherence. A strong background in ambulatory oncology and effective communication are essential for success.

Competenze

• Licensure as RN in Michigan required.
• Oncology certification or willingness to obtain within 1 year.
• Chemotherapy/Biotherapy provider card within 6 months.
• MICMT training or equivalent within 60 days.
• BLS certification within 90 days.
• Completion of company orientation program.

Mansioni

• Serve as primary clinical contact for patients, families and caregivers.
• Coordinate diagnostic work-up and treatment starts across ambulatory oncology services.
• Collaborate with physicians, nurses, pharmacists, social workers and dietitians.
• Educate patients on diagnosis, treatment options, and clinic workflows.
• Assist with prior authorizations, referrals, and record retrieval.
• Address barriers to treatment adherence including transportation and finances.
• Document navigation interventions and patient outcomes in the EHR.
• Perform telephone triage for urgent symptoms and escalate as needed.
• Coordinate follow-up and high-risk patient outreach with the care team.
• Support quality improvement and care coordination initiatives with leadership.
