Job summary

Mercy is seeking a Registered Nurse (RN) Nurse Navigator for the Oncology program. The role centers on guiding oncology patients and their families through the cancer care continuum, coordinating care with physicians, infusion services, imaging, and supportive teams.

You will educate patients on diagnoses and treatment plans, address barriers to care, and document progress in EPIC, contributing to high-quality, timely care and an exceptional patient experience.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in state of practice.
• 2–3 years of nursing experience.
• BLS certification.
• BSN preferred.
• Oncology/infusion or care coordination experience desired.
• OCN certification or willingness to obtain preferred.
• ACLS certification desirable.

Responsibilities

• Coordinate and navigate oncology care across diagnosis, treatment, survivorship, and follow-up.
• Educate patients and families on diagnoses, treatment plans, medications, procedures, and side effects.
• Facilitate communication among patients, providers, infusion services, imaging, and support teams.
• Collaborate with physicians and interdisciplinary teams to implement individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and address barriers to care.
• Provide emotional support and link to community resources and financial assistance.
• Maintain timely documentation in EPIC and other systems.
• Track outcomes and contribute to program development.
