Job summary

Covenant Health Cancer Center is seeking a Nurse Navigator to provide clinical support, treatment access, education and coordination of patient-centered care for cancer patients and their families.

The role requires at least three years of oncology nursing experience, TN RN licensure, and a focus on guiding patients through diagnosis, treatment options, and supportive services within a comprehensive cancer program.

Qualifications

• Minimum three years oncology experience.
• TN RN license required; Oncology certification preferred within three years of employment.
• Oncology certification preferred at time of hire or working toward within three years of employment.

Responsibilities

• Serve as the essential link between patients and other care providers to provide resources and assistance with accessing clinical and supportive care services within the healthcare system and the community.
• Educates patients about their diagnosis, treatment options, and resources including Clinical Trials, genetic counseling, or fertility preservation.
• Educates patients about their disease, prescribed treatment, and side effects, and helps them make informed decisions.
• Advocates on behalf of the patient.
• Oversees and participates in all multidisciplinary cancer conferences.
• Follows patients throughout the continuum of care per department policies and procedures.
• Uses Oncology nursing skills to obtain medical history and plan navigation entry.
• Aids in coordination of care between providers and facilities as needed.
• Facilitates patient appointments with labs, diagnostic areas, and specialty physicians.
• Addresses barriers to care and referrals to appropriate support services.
• Evaluates and reports quality metrics at quarterly Cancer Committee meetings.
• Completes Survivorship Care Plans for eligible patients post-treatment and records in the chart.
