Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the applicable act.
• Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and organ function.
• Collaborate with hematology/oncology teams for inpatient and outpatient care.
• Prepare investigational and standard-of-care medications for clinical trials.
• Attend hematology/oncology rounds and provide pharmacy services in the their units.
