Qualifications

• Licensed Pharmacist certification (IDFPR) required.
• Bachelor’s Degree in Pharmacy required.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring.
• Medication Therapy Management (MTM): Provide MTM services and patient education.
• Dosing Recommendations: Determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams (inpatient and outpatient).
• Prepare investigational and standard care medications for clinical trials safely and on time.
• Attend hematology/oncology rounds and provide pharmacy services in the interdisciplinary team.
• Collaborate with providers to enhance patient-centered care and drug therapy management.
• Oversee chemotherapy preparation and participate in transitions of care and staff development.
