Job summary

Carle Health is seeking a pharmacist to deliver comprehensive clinical pharmaceutical care to oncology patients within a multidisciplinary team. You will participate in patient rounds, provide MTM and dosing recommendations, and support safe preparation of chemotherapy and investigational therapies.

The role involves collaboration with hematology/oncology providers, inpatient and outpatient services, and participation in medication policies and safety initiatives.

Qualifications

• Licensed Pharmacist with IDFPR certification.
• Bachelor’s Degree in Pharmacy required.
• Clinical work experience preferred.

Responsibilities

• Direct patient care to optimize prescribing, administration and monitoring.
• Provide MTM services and patient education.
• Determine dosing based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Prepare chemotherapy and investigational meds safely and timely.
• Assist with transitions of care and research activities.
