Job summary

Qualifications

• Must be a graduate from an accredited school of Nursing.
• RN license issued by the State of California.
• BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.
• Minimum 3-5 years of demonstrated oncology (inpatient or outpatient) care coordination experience.
• Excellent interpersonal skills and verbal and written communication to collaborate with all levels of staff, patients, and families.
• Ability to work effectively and cooperatively with team members across multiple departments.
• Commitment to lifelong learning (e.g., professional certification, clinical ladder advancement).

Responsibilities

• Act as the primary bridge between inpatient teams and outpatient oncology services.
• Coordinate scheduling of post-discharge appointments including clinic visits and infusions.
• Review discharge summaries to ensure accurate transition of information.
• Perform daily inpatient oncology census review and identify discharge readiness barriers.
• Collaborate with clinicians, case managers, social workers, nursing, pharmacy and support services.
• Address barriers to discharge and follow-up care (authorization, transportation, social determinants).
• Serve as a clinical resource on oncology treatment pathways and coordination requirements.
• Educate patients on follow-up plans and discharge instructions for oncology care.
• Communicate critical information to outpatient teams to ensure continuity and safety.
• Track post-discharge follow-through and escalate gaps as needed.
• Partner with case management and social work for complex discharge planning.
• Participate in quality improvement initiatives to reduce delays and readmissions.
