Job summary

UC San Diego Health is seeking a Discharge Coordinator RN to bridge inpatient and outpatient oncology services, coordinating post-discharge needs including clinic visits, infusions, and radiation therapy.

You will review care plans, monitor discharge readiness, and collaborate with case management, social work, nursing, and pharmacy to ensure timely, safe transitions and optimal patient outcomes.

Qualifications

• Must be a graduate from an accredited school of Nursing.
• RN license issued by the State of California.
• BART or BLS at time of hire with commitment to get BART within six months of hire date.
• Minimum 3–5 years oncology care coordination experience.
• Excellent interpersonal skills and verbal and written communication to collaborate with all levels of staff, patients, and families.
• Ability to work effectively and cooperatively with team members across multiple departments.
• Excellent critical thinking, clinical and patient teaching skills with vulnerable patient population.
• Commitment to lifelong learning (e.g., certification, clinical ladder).
• BSN or MSN (preferred).
• Oncology Certification (OCN) (preferred).
• Two or more years of outpatient oncology experience (preferred).
• Two or more years of oncology infusion experience (preferred).

Responsibilities

• Act as the primary bridge between inpatient teams and outpatient oncology services.
• Coordinate and facilitate timely scheduling of post-discharge appointments.
• Review discharge summaries and plans of care for complete transition of information.
• Perform daily inpatient oncology census to manage discharge readiness and barriers.
• Collaborate with clinicians, case managers, social workers, nursing, pharmacy, and support services.
• Identify and address barriers to discharge and follow-up care.
• Serve as a clinical resource on oncology pathways and coordination requirements.
• Educate patients on follow-up plans and discharge instructions.
• Communicate critical clinical information to outpatient teams.
• Track post-discharge follow-through and escalate gaps as needed.
• Partner with case management and social work for complex discharge planning.
• Participate in quality improvement to reduce delays and readmissions.
