Job summary

Cleveland Clinic Main Campus is seeking a Care Coordination Team Manager to bridge clinical care strategy with the Clinical Care Coordinators, ensuring standardization and programmatic consistency across locations.

The role interfaces with nursing, market and network services, payer contacts, information technology and enterprise care coordination. It requires a minimum of two years of leadership experience and a Monday–Friday schedule with travel to various sites.

Qualifications

• Bachelor's of Science in Nursing (BSN) required.
• Current RN licensure in the state.
• Five years of nursing experience and one year in care coordination or case management.
• Supervisory or management experience.
• BLS certification through AHA or Red Cross.

Responsibilities

• Monitor Care Coordination team's competencies and outcomes.
• Serve as liaison between payer programs, clinics, and care coordination teams.
• Act as information and education resource for the interdisciplinary team.
• Maintain procedure manual and communicate changes to leadership.
• Ensure workflow aligns with enterprise care coordination agreements.
• Coordinate care for high-risk patients and optimize outcomes.
• Ensure even distribution of patient load and effective panel management.
• Lead in hiring and onboarding of new nurses with the team.
• Coach and mentor care coordinators and contribute to APRs.
