Job summary

Mount Sinai Health System is seeking a Clinical Coordinator to lead coordination of specialty patient care with nursing, social work, care management and other disciplines. This role focuses on developing care plans, education and program objectives across multiple departments.

The position requires a BSN and at least 2 years in oncology, with NYS licensure and BCLS certification. Strong leadership and advocacy for patients and families are essential.

Qualifications

• Bachelors degree in Nursing required; Masters preferred.
• 2 years oncology experience.
• Current NYS nurse licensure.
• BCLS certification.
• Collective bargaining unit: NYSNA-MSH.

Responsibilities

• Coordinate clinical care within a designated specialty with multi-disciplinary teams.
• Lead day-to-day patient care coordination per program objectives.
• Provide consultation and education to the interdisciplinary team.
• Promote an interdisciplinary approach to patient care delivery.
• Act as a patient advocate and resource to patients and families.
• Participate in quality improvement and policy development.
