Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state RN licensure and BLS certification.
• Three to five years of nursing experience relevant to oncology care.
• Cancer-specific certification or ongoing qualifying education if oncology work.
• Ability to coordinate care across multidisciplinary teams.

Responsibilities

• Coordinate care and disease management longitudinally for high-risk oncology patients.
• Outreach to patients to improve outcomes and reduce ED visits/readmissions.
• Collaborate with multidisciplinary teams across the continuum of care.
• Conduct comprehensive clinical assessments including medical and psychosocial needs.
• Serve as liaison and advocate for patients and families; assist transitions of care.
