Job summary

Cleveland Clinic is seeking an RN Care Coordinator at the Main Campus in Cleveland, OH, to partner with a multidisciplinary team to coordinate care for high‑risk patients and improve outcomes.

You will conduct comprehensive assessments, manage disease plans, educate patients and families, and help navigate transitions across settings while linking to community resources.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• BSN strongly preferred.
• 3–5 years of nursing experience.
• Current state RN licensure.
• BLS certification required.
• Specialty certification preferred.
• Cancer-specific certification may be required for certain roles.

Responsibilities

• Collaborate with a multidisciplinary care team to set goals and plan interventions.
• Provide care and disease management coordination.
• Identify patients needing ongoing coordination and conduct outreach.
• Perform comprehensive clinical assessments including medical, behavioral, and social needs.
• Inform patients and families about care coordination.
• Monitor adherence to care plans and perform reassessments.

What You'll Do

Requirements
