JOL Healthcare seeks a Hospice RN Case Manager to coordinate high-quality hospice care across patient homes and facilities. You will develop care plans, manage visits, and work with an interdisciplinary team to ensure regulatory compliance and compassionate support throughout the hospice journey.

Requires an RN license, 2+ years nursing experience with hospice/palliative care preferred, and ability to travel regionally. EMR proficiency and strong communication are essential for success.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• Hospice and Palliative Care certification preferred.
• Minimum 2 years nursing experience, 1 year in hospice/palliative care preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Proficiency with EMR systems and documentation.
• Ability to work independently and manage a caseload.
• Compassionate approach to patient and family care.

Responsabilités

• Conduct comprehensive assessments of hospice patients.
• Develop and implement individualized care plans.
• Coordinate care with physicians, social workers, chaplains and others.
• Provide skilled nursing care and symptom management.
• Educate patients and families on disease progression and care expectations.
• Document all patient interactions and care in EMR system.
• Participate in interdisciplinary team meetings and care planning.
• Ensure compliance with hospice policies and regulatory requirements.
• Respond to patient and family needs in a timely and compassionate manner.
