Job summary

Qualifications

Responsibilities

• Contributes to department goals and adheres to policies and safety standards.
• Collaborates with multidisciplinary team to coordinate care plan.
• Communicates with patient, family, and care team to address goals.
• Develops care plans with short/long-term objectives and timeframes.
• Evaluates patient response and modifies plans as needed.
• Maintains knowledge of regulatory requirements for admission and reimbursement.
• Manages caseload and coordinates resources for plan execution.
• Participates in performance and service improvement initiatives.
• Performs comprehensive assessments to identify problems and needs.
