Job summary

Senior Care Partners PACE in Kalamazoo, MI is seeking a Registered Nurse Care Coordinator to join an interdisciplinary team providing participant-centered care at the community level. You will coordinate personalized plans, support transitions, and advocate for holistic well‑being of older adults.

The RN will conduct in-home assessments, manage chronic conditions, ensure medication safety, and work closely with families and care partners to promote independence and quality of life.

Qualifications

• Nursing diploma or degree from an accredited school.
• Current RN licensure in Michigan.
• At least 1 year working with frail older adults or in long-term care.
• At least 1 year of home care experience.

Responsibilities

• Coordinate personalized care plans with participants and IDT.
• Advocate for participant preferences, autonomy, and quality of life.
• Perform in-home nursing assessments and immunizations as needed.
• Manage chronic conditions with education and action plans.
• Review medications and educate on safe use and adherence.
• Coordinate transitions of care with hospital and facility partners.
• Document timely nursing care and maintain regulatory compliance.
