Job summary

The Senior Alliance seeks a Registered Nurse with Michigan licensure to assess, plan, and monitor elderly participants' care in collaboration with social workers and care teams.

The role involves coordinating services with physicians, home health agencies, families, and community resources; ensuring documentation and compliance; and supporting program goals across the Michigan aging network.

Qualifications

• Must have excellent interpersonal and group process skills.
• Accurate, conscientious, flexible with attention to detail.
• Familiar with computers and able to document care management activities.

Responsibilities

• Conducts comprehensive participant assessments to determine health care needs.
• Develops participant care plans with the team social worker, including frequency/duration of services.
• Facilitates delivery of services with physicians, home health, and other providers.
• Monitors care plans and reassesses needs for adjustments.
• Collaborates with family members and caregivers to maximize informal support systems.
• Maintains liaisons with hospitals and service providers in the area.
