Job summary

New Freedom is seeking a Re-Entry Specialist to provide case management, advocacy, and transition support for members at the Deer Valley Facility in Phoenix, AZ. You will help identify needs, develop plans, and coordinate services to overcome barriers to housing, healthcare, benefits, transportation, employment, and education.

Qualifications

Bachelor's degree not required; a high school diploma or GED is required.
Experience in case management, care coordination, or reentry services preferred.
Knowledge of community resources and documentation practices is important.

Responsibilities

Develop and monitor individualized service and transition plans.
Coordinate housing, healthcare, benefits, transportation, employment, education, and community resources.
Navigate referrals, applications, appointments, and eligibility processes.
Support recovery, self-sufficiency, and community reintegration.
Collaborate with treatment teams and community providers to address needs and barriers.
