New Freedom is seeking a Re-Entry Specialist to provide case management and transition support for members reintegrating into the community, coordinating housing, healthcare, and employment resources. Qualifications

• High school diploma or GED required; further degree preferred.
• 2+ years in case management, care coordination, or reentry services preferred.
• Experience with EHR systems and community resource coordination is a plus.

Responsibilities

• Develop and monitor individualized service and transition plans.
• Coordinate housing, healthcare, benefits, transportation, employment, education, and community resources.
• Assist members with referrals, applications, appointments, and eligibility processes.
• Support recovery, self-sufficiency, and successful community reintegration.
• Collaborate with treatment teams and providers to address member needs and barriers.
• Maintain timely, accurate case management documentation.
• Assist with approved appointments and transportation as needed.
