Job summary

New Freedom is seeking a Re-Entry Specialist to provide case management, advocacy, and transition support for members at the Deer Valley Facility in Phoenix, AZ. You will help identify needs, develop plans, and coordinate services to overcome barriers to housing, healthcare, benefits, transportation, employment, and education.

Qualifications

About the Role

The Re-Entry Specialist provides case management, advocacy, resource coordination, and transition support to New Freedom members. Working closely with members and multidisciplinary treatment teams, this position helps identify needs, develop individualized plans, coordinate services, and reduce barriers related to housing, healthcare, benefits, transportation, employment, education, and financial stability.

What You'll Be Responsible For
  • Developing and monitoring individualized service and transition plans
  • Coordinating housing, healthcare, benefits, transportation, employment, education, and community resources
  • Helping members navigate referrals, applications, appointments, and eligibility processes
  • Supporting recovery, self-sufficiency, and successful community reintegration
  • Collaborating with treatment teams and community providers to address member needs and barriers
  • Maintaining timely, accurate, and compliant case management documentation
  • Supporting approved appointments, community visits, and transportation activities when assigned

What We're Looking For
  • High school diploma or GED (Associate or bachelors degree in Social Work, Behavioral Health, Psychology, Human Services, Criminal Justice, Public Health, or a related field preferred)
  • Must be 21 years of age or older
  • Behavioral Health Technician designation based on qualifying education and/or experience
  • Experience in case management, care coordination, social services, behavioral health, recovery support, reentry services, or human services (Two or more years of relevant case management, care coordination, reentry, recovery, or behavioral health experience preferred)
  • Knowledge of community resources, individualized planning, service coordination, documentation, and member advocacy
  • Strong communication, organization, time management, and problem-solving skills
  • Ability to manage multiple members, referrals, deadlines, and follow-up responsibilities
  • Ability to maintain confidentiality and appropriate professional boundaries
  • Proficiency with Microsoft Office, Electronic Health Records, case management systems, and related technology (Experience using KIPU or a comparable Electronic Health Record or case management system preferred)
  • Case management training, certification, or demonstrated experience preferred
  • Relevant lived experience with recovery, reentry, behavioral health services, or justice-system involvement and Peer and Recovery Support Specialist certification preferred
  • Experience serving justice-involved individuals or populations facing housing, employment, healthcare, benefits, or transportation barriers preferred
Environment + Physical Demands

This position works across outpatient behavioral health, recovery, reentry, residential, professional office, and community-based environments. Responsibilities may include member meetings, service coordination, community appointments, transportation, and other off-site activities. Scheduling may occasionally include early mornings, evenings, weekends, or holidays based on program and member needs.

Qualifications
Education
  • Required: High School or better.
  • Preferred: Associates or better in Psychology or related field. Bachelors or better in Psychology or related field.
Experience
  • Required: Experience providing case management, care coordination, social services, behavioral health, recovery support, reentry services, or other related human services. Working knowledge of community resources, service coordination, individualized planning, documentation, and member advocacy. Strong communication, interpersonal, organizational, time-management, critical-thinking, and problem-solving skills. Ability to manage multiple members, referrals, deadlines, and follow-up responsibilities in a fast-paced environment. Ability to maintain professional boundaries and appropriately safeguard confidential member information. Ability to work independently while collaborating effectively with a multidisciplinary treatment team. Proficiency with Microsoft Office applications, internet-based platforms, Electronic Health Records, case management systems, and other business technology. Behavioral Health Technician designation required (2+ years behavioral health work experience and/or higher education). Must be 21 years or older.
  • Preferred: 2 years: Case management, care coordination, reentry services, behavioral health, recovery services, or related experience. Case Management certification, training, or demonstrated case management experience. Relevant lived experience with recovery, reentry, behavioral health services, or justice-system involvement and/or Peer and Recovery Support Specialist certification. Experience working with justice-involved individuals or populations experiencing housing, employment, healthcare, benefits, or transportation barriers. Experience with KIPU or a comparable Electronic Health Record or case management system.
