Job summary

Circadia Health is seeking a Nurse Practitioner to join as an on-site, embedded clinician at a Cedar Grove, New Jersey facility. You will provide direct patient care, perform assessments, and guide staff on Circadia integration within care workflows.

Responsibilities include training staff, interpreting AI-driven insights, and ensuring timely documentation. This role demands independence, a startup mindset, and strong collaboration with clinical teams.

Qualifications

• NJ NP license and active nursing license (ANCC)
• 2+ years NP experience with complex adult populations; ability to practice independently
• Valid CA driver’s license; comfortable traveling locally
• Thrives in a dynamic startup environment
• English required; Spanish preferred

Responsibilities

• Direct patient care: conduct initial and follow-up assessments, initiate/adjust plans of care, coordinate with providers
• Tech → bedside integration: train and coach staff, integrate Circadia into clinical workflows
• Data into action: interpret trend-based insights, prioritize high‑risk patients, drive timely interventions
• Close the loop: complete documentation within the same day
• High‑volume rounding: ~25+ visits/day attending new admits and flagged patients
• Quality & product feedback: surface field insights to improve workflows
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams
