Job summary

Circadia Health is seeking a licensed Nurse Practitioner to join our on-site team at Cedar Creek Post Acute in Mt. Juliet, TN. This full-time role blends direct patient care with tech-enabled bedside workflows, AI-assisted documentation, and real-time data interpretation to prevent hospital readmissions.

You will conduct assessments, initiate plans of care, train staff on Circadia integration, and collaborate with clinical ops to optimize care delivery in a busy post-acute setting.

Qualifications

• Must be Certified Nurse Practitioner with current, unrestricted license.
• ANCC certification in Adult/Family/Geriatric/Acute is required.
• 2+ years NP experience with complex adult populations, able to practice independently.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments; initiate/adjust plans of care; coordinate with facility providers.
• Tech bedside integration: train and coach staff on Circadia within clinical workflows.
• Data into action: interpret trend-based insights; prioritize high-risk patients; drive timely interventions.
• Close the loop: complete documentation within the same day.
• High-volume rounding: perform visits across SNFs (approx. 25+/day).
• Quality & product feedback: surface field insights to improve workflows and tech integration.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
