The Kroger Co. is seeking a pharmacist to provide patient-centered services from our centralized contact center at the University of Cincinnati location.

Services include comprehensive medication reviews, targeted interventions, and adherence monitoring, delivered telephonically to patients, pharmacies, and providers, in alignment with Kroger Health values. Responsibilities span medication problem resolution, patient education, and collaboration with the healthcare team.

Qualifications

• Pharmacy degree required and licensure in resident state.
• State Board of Pharmacy license in good standing.
• ASHP-accredited residency or 2+ years relevant pharmacy experience.

Responsabilités

• Identify, prioritize, and resolve medication-related problems and determine appropriate follow-up care.
• Help patients identify barriers to adherence and develop solutions.
• Use professional communication to engage patients and caregivers in decision making.
• Formulate therapeutic goals for patients based on medications and disease states using guidelines.
• Make professional recommendations to other healthcare team members clearly and timely.
• Demonstrate cultural competence and respect for patient preferences and needs.
• Teach, mentor and supervise pharmacy students on IPPEs and APPEs.
• Precept and mentor other staff, interns, or residents.
• Ensure patient information and all areas of practice are compliant.
• Meet productivity and performance expectations set by supervisor.
• Align with Kroger Health’s purpose, vision, and mission.
• Document all services in a timely manner and manage workload efficiently.
• Assist supervisor with other projects as needed and provide support to Kroger Health operations.
• Ability to work in a high-paced and sometimes stressful environment.
• Ability to manage conflict in a collaborative manner.
• Demonstrate honesty and integrity regarding customer and business information.
• Follow directions and seek help to resolve customer and business issues.
• Provide support with minors, individuals with special needs, and older adults.
• Must be able to perform essential functions with or without reasonable accommodation.
