Job summary

Hebrew SeniorLife is seeking a Licensed Practical Nurse (LPN) to work with a Registered Nurse to provide high-quality nursing care for residents in an on-site Massachusetts setting.

The LPN will participate in assessment, planning, implementation, and evaluation, support a culture of change, and mentor other team members to maximize patient quality of life and safety. Collaboration with the MDT is essential.

Qualifications

• Graduate of approved school of nursing.
• Current Massachusetts license as an LPN.
• Previous experience preferred.
• Commitment to quality care and patient dignity.

Responsibilities

• Ensures all care delivered is within the scope and established standards of safe professional nursing practice with core care competencies. Collaborates with the multidisciplinary team to meet the clinical needs of the patient and monitor outcomes of care.
• Adheres to best practice recommendations and implements evidence-based interventions to ensure optimum outcomes for patients.
• Practices in a primary nursing model and maintains accountability for a set of assigned patients with RN oversight of assessment and care planning.
• Documents pertinent information regarding care delivered in designated areas of patient’s record using the EMR.
• May be assigned charge nurse responsibilities with RN oversight; participate in rounds, audits, and staff scheduling.
