Job summary

Hamilton Health Box is seeking an On-Site Clinician (OSC) LPN/LVN to manage daily clinical and administrative operations at our Hobart, OK location. You will interact with employees for scheduling and follow-up, deliver protocol-driven care, and support preventive health education and screenings.

The OSC may coordinate care with pharmacy services and external referrals, build relationships locally, and travel between HHB clinics as needed. BLS and LPN licensure are required prior to onboarding.

Qualifications

• Licensed Practical Nurse license required.
• BLS Certification required prior to onboarding.

Responsibilities

• Collaborate with other departments to ensure employees receive appropriate care.
• Formulate programs based on health risk assessment results and target high-need areas.
• Assist with annual physicals, screenings, testing, and results reporting.
• Schedule follow-up exams, return-to-work exams, and consultations.
• Coordinate client referrals and document progress in medical records.
