Job summary

Marion County Health & Human Services seeks a Services Coordinator for the Intellectual and Developmental Disabilities (I/DD) team to provide case management to individuals with developmental disabilities across home and community settings.

The role requires clear communication, policy-driven planning, and the ability to facilitate meetings and coordinate with families, providers, and partners. A driver's license and field travel are expected in a dynamic, on-site work environment.

Qualifications

• Five years of education in a related field and/or paraprofessional experience/training serving individuals with developmental disabilities.
• Knowledge of the public service system for developmental disability services in Oregon.
• Any satisfactory combination of work, education, training, or experience as determined by Marion County.

Responsibilities

• Facilitate intake and eligibility determination and develop an Individual Support Plan for individuals receiving K‑plan funded services.
• Coordinate with schools and community partners to ensure safety and services; attend IEP meetings as needed.
• Maintain client records and document case management activities per policy; monitor services per ODDS guidelines.
