Job summary

Damar Services, a mission-driven non-profit in Indiana, seeks a dedicated Case Manager to lead treatment services for individuals with intellectual and developmental disabilities and autism. You will develop individualized IPPs for new admissions, coordinate with residential staff, and ensure timely documentation and client progress reporting.

The role emphasizes collaboration with families, clinicians, and educators, with strong emphasis on IPP updates, assessments, and quarterly meetings.

Qualifications

• Minimum one year experience with DD/ID population or equivalent training
• Must meet one of the listed degree/experience combinations
• Must be 21+ years old and hold Indiana driver’s license
• Clear background and pass pre-employment screen

Responsibilities

• Lead, direct, and monitor treatment services for clients
• Develop and update IPPs within 7 days of admission and maintain monthly updates
• Represent clients at court reviews and IEP/Case Conferences with accurate progress information
• Coordinate with Residential Managers to ensure implementation of programs
• Maintain case notes, data sheets, and weekly family and referral communications
