Job summary

Pine Street Inn is seeking a Supportive Housing Case Manager to provide intensive, on-site case management for chronically homeless individuals in Boston. You will work with tenants using Housing First, Harm Reduction, and Motivational Interviewing to build stable housing and access benefits, health care, education, and employment opportunities.

The role requires strong written communication, proficiency in Microsoft Office, and the ability to collaborate with a multidisciplinary team.

Qualifications

• Strong written communication skills.
• Proficiency with Microsoft Office products.
• Bi-lingual – Spanish/English preferred.
• Undergraduate degree in a related field (e.g. social work, psychology, education, nursing).
• Experience with substance abuse, mental illness, trauma, homelessness or related complex disorders.
• Knowledge of government benefits systems and local resources.
• Experience with HMIS.

Responsibilities

• Provide intensive case management for tenants using Housing First, Harm Reduction and Motivational Interviewing.
• Assist tenants with required program documentation and obtaining benefits, health care, education, employment, and legal aid.
• Develop individual service plans and maintain necessary documentation.
• Collaborate with Housing staff on program-wide efforts to stabilize tenants and promote growth.
• Work on-site 100% and participate as part of a multidisciplinary team.
