New York Community Care is seeking a compassionate Care Manager to join our growing Assisted Living Center. The role focuses on coordinating client care, supporting patients and families, and ensuring high-quality home care delivery while complying with state guidelines.

The ideal candidate will coordinate services, conduct assessments, communicate with healthcare providers, and oversee caregiver assignments in a full-time, in-person role.

Qualifications

• RN license required to practice in home care.
• Strong communication with patients, families, and healthcare providers.
• Ability to assess needs and develop individualized care plans.
• Ability to schedule and oversee caregiver assignments and service hours.

Responsibilities

• Coordinate and manage home care services for assigned clients.
• Conduct client assessments and develop individualized care plans.
